I APPRECIATE IT. THIS PROGRAM AND WE DID SEND, I THINK, AHEAD OF TIME PROBABLY TOO MUCH INFORMATION AS FAR AS THE PROGRAM PLAN AND A FIVE PAGE BUDGET FOR THE $160,000 GRANT THAT WE GET FOR CAROLINE COUNTY TO ADMINISTER THIS PROGRAM FOR OUR VULNERABLE OLDER ADULTS, AND JUST TO GIVE YOU A BACKGROUND IN THE REASON, THE REASON WHY WE DO THAT IS THE SENIOR CARE PROGRAM IS FUNDED BY DEPARTMENT OF AGING THROUGH STATE FUNDS.
IT'S BEEN IN EXISTENCE SINCE THE EARLY 80S, AND AGAIN, THE INTENT IS TO HELP SUPPORT THE OLDER, VULNERABLE, OLDER ADULTS THAT ARE 65 AND OLDER IN EACH OF THE COUNTIES.
EVERY COUNTY IN THE STATE HAS A SENIOR CARE PROGRAM, AND THE FUNDING COMES DIRECTLY TO THE AAAS OR THE AREA AGENCY ON AGING, AND UPPER SHORE IS THE AAA FOR KENT, CAROLINE, KENT AND TALBOT COUNTY AND THEY EACH HAVE A PROGRAM THAT IS UNIQUE.
SO THE PURPOSE OF THE SENIOR CARE PROGRAM IS TO PROVIDE SERVICES AND SUPPORTS FOR ADULTS 65 OR OLDER LIVING IN THE COMMUNITY WHO QUALIFY BASED ON THEIR INCOME ASSETS AND WHO HAVE FUNCTIONAL DISABILITIES THAT PUT THEM AT RISK OF NURSING HOME PLACEMENT.
THE PROGRAM PROVIDES SERVICES AND SUPPORTS TO HELP THEM LIVE IN THEIR OWN HOMES.
EVERY AGAIN EVERY COUNTY IN MARYLAND RECEIVES FUNDS THROUGH THEIR AREA AGENCY ON AGING.
THE PROGRAM DOES ALLOW FOR A HIGH DEGREE OF FLEXIBILITY IN DESIGN BASED ON COUNTY RESOURCES AND NEEDS, AND THIS THIS PROGRAM IS TRULY A PARTNERSHIP BETWEEN OUR AGENCY WHO ADMINISTERS IT, AND OUR OTHER PUBLIC ENTITIES, THE LOCAL HEALTH DEPARTMENT, DSS, AND ALSO THE COUNTY GOVERNMENT.
THROUGHOUT THE STATE THERE'S THERE'S DIFFERENT MODELS IN THE STATE OF MARYLAND.
FOR THIS PARTICULAR PROGRAM, AND GENERALLY THEY'RE EITHER ADMINISTERED DIRECTLY OUT OF THE AGENCY ON AGING, SOMETIMES OUT OF THE DEPARTMENT OF SOCIAL SERVICES AND SOMETIMES OUT OF THE HEALTH DEPARTMENT, AND IN CAROLINE. IT'S SORT OF A HYBRID, SORT OF A MIX, AND HOW WE PARTNER WITH THEM.
LET ME SKIP OVER. THERE'S REALLY FOUR CORE SERVICES THAT THE SENIOR CARE PROGRAM PROVIDES, AND THE FIRST SERVICE IS WE SCREEN FOR ELIGIBILITY FOR OTHER AVAILABLE SUPPORTIVE PROGRAMS. SO WE DO WHEN SOMEONE IS REFERRED TO THE PROGRAM WE MEET WITH THEM IN THEIR HOMES AND WITH THEIR FAMILY OR WHOEVER THEY CHOOSE TO HAVE PRESENT AND WE TALK ABOUT HEALTH ISSUES AND THEN WE TALK ABOUT FINANCIAL ISSUES AND RESOURCES, AND IF WE FIND THEY'RE ELIGIBLE FOR OTHER PROGRAMS OTHER THAN SENIOR CARE SPECIFICALLY, SOMETIMES MEDICAID, AND THEY DON'T KNOW THAT THEY'RE ELIGIBLE FOR SOME SERVICES, WE LINK THEM UP SENIOR CARE DOLLARS OR DOLLARS OF LAST RESORT TO BE USED WHEN ALL OTHER SERVICES HAVE BEEN EXPLORED AND THEY'RE STILL IN NEED IN THAT HOME.
THERE'S ALSO A COMPREHENSIVE ASSESSMENT OF THE INDIVIDUAL NEEDS, AND IN CAROLINE COUNTY.
OUR PARTNERS AT THE LOCAL HEALTH DEPARTMENT ARE THE ONES THAT WILL COME OUT AND DO A VERY SPECIFIC ASSESSMENT, VERY IN-DEPTH. THEY'LL COME TO THE HOME AND TO LOOK AT WHAT THAT PERSON NEEDS TO STAY HEALTHY AND SAFE IN THEIR HOME.
WE HAVE ONE FULL TIME CASE MANAGER PAID FOR BY THE GRANT, AND SHE IS IN THE HOMES.
IT'S ONE OF THE FEW PROGRAMS OUT THERE WHERE WE ARE.
WE DON'T DO A LOT OF TELEPHONE CASE MANAGEMENT, ALTHOUGH WE DO THAT TOO, BUT SPECIFICALLY, WE ARE IN THE HOMES MEETING WITH THE FAMILIES, SEEING WHAT'S GOING ON, AND IT'S REALLY AN EYE OPENER FOR SOME OF OUR OLDER ADULTS WILL GO

TO THEIR PHYSICIANS AND SAY, YOU KNOW, YES, I'M DOING FINE.I'M TAKING ALL MY MEDICINES, AND WHEN WE'RE IN THE HOMES, WE'RE SEEING THAT 'S NOT THE CASE.
SO IT'S A LOT OF GOOD FEEDBACK, AND REALLY A HELP FOR A LOT OF PEOPLE, AND THEN LASTLY ONE OF THE, ONE OF THE OTHER CORE SERVICES IS THE MONEY IS USED AS WHAT WE CALL GAP FILLING MONEY.
SO WE DO HAVE GRANT MONEY SET ASIDE SPECIFICALLY FOR THAT, AND WE TRY TO MAKE IT AS EASY AS POSSIBLE FOR THE ELIGIBILITY FOR WHO IS ELIGIBLE FOR THE PROGRAM, THEY WOULD BE CAROLINE COUNTY RESIDENTS 65 AND OLDER WHO HAVE A MODERATE OR SEVERE FUNCTIONAL DISABILITY.
SO THEY'RE HAVING PROBLEMS WITH THEIR ACTIVITIES OF DAILY LIVING, AND THEN THEY MEET THE FINANCIAL ELIGIBILITY REQUIREMENTS, WHICH ARE CURRENTLY ABOUT 300% OF THE FEDERAL POVERTY LEVEL WHICH IS CURRENTLY ABOUT $1,300 A MONTH.
SO 300% OF THAT IS ABOUT $3,900 A MONTH IN TOTAL INCOME, WHICH INCLUDES SOCIAL SECURITY AND PENSIONS, AND THEY HAVE TO HAVE ASSET LIMITS OF LESS THAN 11,000, AND THAT'S LIQUID ASSETS.
THAT'S NOT HOME VEHICLE, THAT SORT OF THING. AGAIN, THE PARTNERSHIP IS REALLY THE KEY TO THIS, YOU KNOW, NO ONE PROGRAM, NO ONE GRANT, ESPECIALLY $160,000 IS GOING TO SOLVE EVERYBODY'S PROBLEMS. SO WE DO PARTNER, LIKE I WAS SAYING, WITH THE HEALTH DEPARTMENT AND DEPARTMENT OF SOCIAL SERVICES, YOU KNOW, THE COUNTY HELPS BY SUPPORTING THE AREA AGENCY ON AGING.
OTHER AGENCIES WE PARTNER WITH UNIVERSITY OF MARYLAND AT CHOP TANK HEALTH, MEDSTAR BEHAVIORAL HEALTH YOU KNOW, CRITICAL REPAIR. THERE'S A LOT OF AGENCIES, ENTITIES, CHURCHES, CIVIC GROUPS THAT HELP OUT IN THE COUNTY THAT CONTRIBUTE TO THE PROGRAM.
IT'S VERY CLIENT AND FAMILY CENTER TO PLAN OF CARE.
WHAT DO THEY SEE IS THE GREATEST NEEDS? WHAT DO THEY NEED HELP WITH? AND THEN WE TRY TO MAKE THAT HAPPEN AS BEST WE CAN WITH THE DOLLARS THAT WE HAVE, AND ALSO TO CONNECT THEM WITH EVERYBODY ELSE THAT I MENTIONED.
TRY TO POOL OUR RESOURCES TOGETHER AND TO PREVENT OVERLAPS.
WE'VE SERVED 67 SENIORS IN CAROLINE COUNTY SO FAR THIS YEAR.
WE. 71% OF OUR CASELOAD ARE FEMALE, AND I GUESS THAT GOES WITH THE STATISTICS THAT THE WOMEN TEND TO LIVE LONGER THAN MEN, AND SOMETIMES ALSO WHEN A SPOUSE IS LOST, SOME, YOU KNOW, THE FEMALES SOMETIMES HAVE MORE DIFFICULTY MAKING ENDS MEET.
THERE'S SOME FINANCIAL IMPLICATIONS. 40% ARE BLACK OR AFRICAN AMERICAN.
60% OF OUR CASELOAD ARE 80 YEARS OLD OR OLDER.
47% LIVE ALONE AND 47% ARE LIVING BELOW THE FEDERAL POVERTY LEVEL OF 1304 A MONTH.
JUST TO THROW SOME NUMBERS OUT OF THE $160,000 GRANT IF WE, YOU KNOW, DIVIDE THAT BY THE NUMBER OF CLIENTS WE SERVE. WE'RE SERVING PEOPLE ABOUT $2,400 PER PERSON PER YEAR IS WHAT WE'RE PROVIDING TO THEM, AND IF YOU LOOK AT WHAT NURSING HOME COSTS ARE FROM STATISTICS FROM 2022 AVERAGE NURSING HOME COSTS IN MARYLAND WERE $340 A DAY. SO THAT'S ABOUT $300 AND, YOU KNOW, $124,000 A YEAR TO KEEP SOMEONE IN A NURSING HOME ON MEDICAID VERSUS MAYBE $2,400 A YEAR TO TRY TO KEEP THEM OUT OF A NURSING HOME.
IF WE CAN DEFER GOING INTO A NURSING HOME OR TAXING OUR HEALTH CARE SYSTEM.
AS FAR AS THE GRANT ALLOCATION, 10% OF OUR FUNDS ARE GO TO ADMINISTRATION.
THERE'S A LOT OF PURCHASING AND, EXCUSE ME, OVERSIGHT OF FUNDS AND THINGS LIKE THAT.
37% GOES FOR THE STAFF, FOR THE CASE MANAGER, AND THEN 53% ARE FOR WHAT WE CALL THE GAP FILLING FUNDS TO FOR DIRECT SERVICES, FOR SUPPLIES AND SERVICES FOR THE SENIORS. WHAT DO WE SPEND THE MONEY ON?

ABOUT 32% OF THOSE GAP FILLING FUNDS ARE USED FOR MEDICAL SUPPLIES, AND THAT DOES INCLUDE INCONTINENCE SUPPLIES, EQUIPMENT NUTRITIONAL SUPPORT [INAUDIBLE] WHEN A PHYSICIAN ORDERS IT BECAUSE THERE'S NO WAY FOR FOLKS TO PURCHASE THAT OTHER THAN, YOU KNOW, WITH THEIR OWN MONEY. 26% OF THE GRANT IS USED FOR PERSONAL CARE SERVICES TO HAVE SOMEONE COME INTO THE HOME AND HELP THEM WITH THEIR BATHING AND HYGIENE.WE USE 10% FOR MEDICAL DAYCARE CO-PAYS TO ALLOW PEOPLE TO GO, BUT CAN'T AFFORD IT.
A BIG CHUNK OF OUR MONEY GOES TO PERSONAL EMERGENCY RESPONSE SYSTEMS. YOU KNOW, THE BUTTONS THAT THEY WEAR OR BRACELETS. 22%, AND THEN 10% FOR SURE, AND TRANSPORTATION SERVICES.
SO THOSE ARE SOME OF THE THINGS THAT WE ARE ABLE TO PAY FOR OUT OF THOSE GAP FILLING FUNDS.
THE MOST REQUESTED SERVICES WE'RE SEEING IS, IS SOMEONE TO COME IN, JUST SOMEONE TO COME IN THE HOME AND HELP IN THE HOME, EITHER WITH CHORES. OBVIOUSLY, TRANSPORTATION, AS YOU WELL KNOW, CAN BE AN ISSUE FOR A LOT OF FOLKS, AND AND THEN PERSONAL CARE. SO THESE ARE THE THINGS THAT WE TRY TO HELP WITH.
AGAIN, OUR LITTLE BIT OF MONEY DOES NOT MAKE A GREAT DIFFERENCE IN AND OF ITSELF, BUT SOMETIMES IF WE CAN HELP IN ONE AREA, THEN THE FAMILY OR THE CLIENT THEMSELVES OR ANOTHER AGENCY CAN COME IN AND HELP PICK UP THE REST.
IT'S A WRAPAROUND TYPE OF SERVICE SYSTEM. LET'S SEE.
FOR CHALLENGES, WE WERE LEVEL FUNDED. I THINK I DID SEND THE FUNDING DISTRIBUTION SHEET FOR THE STATE, AND WE ARE LEVEL FUNDED AT $160,000 FROM FISCAL YEAR 25 TO FISCAL YEAR 26.
WE WERE AT RISK THIS YEAR IN THE SPRING, WHEN OUR GOVERNOR WAS TRYING TO REDUCE THE OVERALL EXPENDITURES IN THE BUDGETS, AND WE WERE TOLD WE WERE GOING TO LOSE MONEY THIS YEAR, WHICH WAS DISTURBING BECAUSE WE WERE ALREADY THREE QUARTERS INTO THE YEAR, AND WE ENDED UP A LOT OF PEOPLE WROTE IN AND ADVOCATED, AND WE WERE ABLE TO HANG ON TO OUR FUNDING FOR THIS YEAR LEVEL FUNDING FOR NEXT YEAR.
IN FISCAL YEAR 27, THERE'S BEEN SOME NEW LEGISLATION PROPOSED BY DEPARTMENT OF AGING THAT'S GOING TO CONSOLIDATE SENIOR CARE AND TWO OTHER SENIOR PROGRAMS. ONE IS IS AN ASSISTED LIVING SUBSIDY, AND THE OTHER IS CONGREGATE HOUSING, WHICH WE DON'T HAVE ANY ON THE SHORE, BUT IT'S COMMON ON THE WESTERN SHORE. SO IT'S GOING TO BE ONE PROGRAM.
UPPER SHORE AGING WILL GET ONE DISTRIBUTION OF FUNDS AND THEN DETERMINE HOW TO ALLOCATE THAT, BUT CERTAINLY OUR SENIOR CARE PROGRAM IS IS OUR.
MORE ROBUST PROGRAM AS FAR AS THE NUMBERS OF PEOPLE THAT WE SERVE, AND AGAIN, THE BIGGEST CHALLENGE FOR US IS INCREASED REFERRALS. WE GET REFERRALS EVERY WEEK. MOST OF THEM COME FROM THE AGENCIES THEMSELVES, LIKE SOCIAL SERVICES, HEALTH DEPARTMENT, FROM OUR SENIOR CENTER SOMETIMES FROM MEDICAL PROVIDERS, FROM EMS WHO ARE OFTEN IN THE HOMES WE SEE SEE SOME DIRE SITUATION SOMETIMES SOME OF OUR OLDER ADULTS ARE IN.
THE OTHER THING IS THE INCREASED COST OF SERVICES AND SUPPLIES.
IT WASN'T A FEW YEARS AGO WE COULD GET SOMEONE A CASE OF INSURE FOR $20 A CASE.
NOW THEY'RE DOUBLE THAT. INCONTINENCE SUPPLIES ARE MORE EXPENSIVE.
THE COST OF FINDING A PROVIDER TO GO IN AND PROVIDE PERSONAL ASSISTANCE IS IS GONE UP TREMENDOUSLY.
WE DO HAVE SOME FLEXIBILITY. AGAIN, THIS PROGRAM HAS ALWAYS ALLOWED THAT WHICH HAS BEEN A WONDERFUL PART OF IT, AND THAT WE ARE ABLE TO PROVIDE. IF A FAMILY HAS A FRIEND OR SOMEONE THAT THEY KNOW CAN COME IN AND DO THE CHORES AND THEY CAN PAY THEM DIRECTLY, WE CAN OFFER THEM, LIKE MANY GRANTS WHERE WE CAN PAY THEM DIRECTLY, AND THEN THEY CAN PAY THEIR PROVIDER OF CHOICE.
SO AGAIN, THIS IS ONE OF THE PROGRAMS I THINK DOES THE MOST FOR SOME OF THE MOST VULNERABLE FOLKS IN THE COUNTY, AND WE'RE HERE TODAY JUST TO KIND OF LET YOU KNOW ABOUT THAT PROGRAM AND ALSO TO ASK FOR YOUR APPROVAL AND TO GO AHEAD AND ADMINISTER THE PROGRAM FOR FISCAL YEAR 26, AS DESCRIBED.

BEING TO THE MEETINGS THAT YOU GUYS STRETCH EVERY DOLLAR THAT YOU HAVE, AND I ALWAYS ADVOCATE FOR, YOU KNOW, SOMETIMES UPSTAGING IS THE ONLY TIME THAT THE SENIORS EVEN GET TO SEE SOMEBODY, AND I GUESS, YOU KNOW, IT GIVES THEM A BIT OF HOPE OR, YOU KNOW, WHERE THEY DON'T HAVE TO GO INTO ASSISTED LIVING, AND , BUT I SUPPORT, YOU KNOW, RAISING I KNOW YOU'RE DOING THE BEST YOU CAN, AND I KNOW SOMETIMES THE SENIORS GET FORGOTTEN WITH THE WITH THE MAJOR BUDGETS, WITH THE STATE AND ALL THAT, AND IT'S TROUBLING BECAUSE PEOPLE WORK THEIR WHOLE LIVES AND WHEN THEY GET TO THE SENIOR LEADERS, SOMETIMES THEY FEEL LIKE THEY, YOU KNOW, THEY'RE FORGOTTEN.SO NO I'M FINE WITH THIS. OH YEAH. SO IS IS UPPER TRADING BASICALLY LIKE A PROVIDER OF LAST RESORT? LIKE THERE'S NOWHERE ELSE SOCIAL SERVICES OR THE HEALTH DEPARTMENT CAN'T CAN'T HELP OUT IN THESE PARTICULAR SITUATION.
SO THE LAST RESORT FOR A SENIOR IS TO TRY TO SEEK HELP FROM UPPER SHORE AGING .
IS THAT IS THAT KIND OF WHAT I'M HEARING? ABSOLUTELY, AND WE MEET MONTHLY.
OUR GROUP MEETS MONTHLY WITH THE HEALTH DEPARTMENT, SOCIAL SERVICES TO TALK ABOUT CASES, AND I DIDN'T MENTION THAT ONE OF THE UNIQUE THINGS IN CAROLINE AND I SHOULD HAVE MENTIONED IS THAT SOCIAL SERVICES HAS BEEN A STRONG PARTNER AND THAT THEY HELP BY PROVIDING THEY HAVE MONEY FOR WHAT THEY CALL IHAS, IN-HOME AID SERVICE, AND IF THEY HAVE CLIENTS THAT ARE GETTING IN-HOME AIDE SERVICES THAT THEY MANAGE AND THEY BECOME, THEY ARE ALSO ELIGIBLE FOR SENIOR CARE FOR SOME ADDITIONAL HELP.
WE PARTNER AND THEY PROVIDE THE CASE MANAGEMENT FOR THOSE CLIENTS, WHICH TAKES THE BURDEN OFF OUR ONE CASE MANAGER, AND IT HELPS US STRETCH OUR DOLLARS EVEN FURTHER, BECAUSE THEN THEY THEY ARE THEY CAN PROVIDE THE IN-HOME AIDE SERVICE, AND WE MIGHT PROVIDE THE EMERGENCY RESPONSE SYSTEM OR A NUTRITIONAL SUPPLEMENT OR SOMETHING ELSE SO WE CAN WORK TOGETHER, AND THEY REALLY SAVE US A LOT OF MONEY BY DOING THAT AND ALSO PREVENTS DUPLICATION.
